Home Health Agency Owners Sentenced to Prison, Ordered to Pay $14.1M for Fraud

It’s been a busy month so far for home health care and hospice fraud.

On Thursday, the U.S. Department of Justice announced that two Detroit-area home health agency owners were sentenced to a combined 16 years of prison for their roles in a multimillion dollar scheme to defraud Medicare by billing for home health services that were never provided.

Hafiz Tahir, 49, and Tasneem Tahir, 44, both of Brownstown, Michigan, were sentenced by U.S. District Judge Marianne O. Battani of the Eastern District of Michigan to 10 and six years in prison, respectively. Judge Battani also ordered the defendants to pay restitution in the amount of $9,674,575 and $4,447,667, respectively, jointly and severally with their co-conspirators.

More than $8 million combined in money judgments were also levied against the two.

Optima Healthcare Solutions — an Alpharetta, Georgia-based company that provides cloud-based billing, clinical and operational software solutions for post-acute care providers — asked hospice providers about their confidence in surviving federal audits and compliance obligations as part of a new survey released on Tuesday. More than 170 hospice agencies across the country participated in the survey, put together with the help of independent research firm Inquisitive Market Research.

Shropshire health bosses spoke about the public health challenge caused by sepsis at a special conference organised by Staffordshire University this week.

Professor Rod Thompson, director of public health, was speaking at the Could it Be Sepsis? event which took place at the Shropshire Education and Conference Centre on the Royal Shrewsbury Hospital site.

He called for a “step change in the way we deal with sepsis” and said not enough of the health service budget was directed towards prevention of serious conditions like sepsis which claimed more UK lives than lung cancer.

Murdered by UNWANTED, UNKNOWINGLY FORCED Palliative Care by St. Joseph's Hospital in Nashua, NH where my husband died of UNTREATED SEPSIS which he contracted there. He was treated for absolutely NOTHING, yet blood tests were done almost daily which show he had an infection and other illnesses that SHOULD have been treated. Medicare FRAUD?????And they never told us anything. I didn't even know he had SEPSIS. The Undertaker told me. I was shocked just as I was when I went through his Medical file. I couldn't and still can't believe what I found. He adamantly stated he wanted treatment and did NOT want to die. So why are these people getting away with ILLEGALLY putting him on Palliative Care and killing him? 
Medicare states he was NOT enrolled in Hospice/Palliative Care, so the claims for payment weren't put in as Hospice/Palliative Care. Because he was put on it ILLEGALLY and they paid the bills. Just a way for Hospitals to get around NON-Consensual, ILLEGAL Hospice/Palliative care! And it worked! Some of the money Medicare is going after, but not ALL of the money which was all paid ILLEGALLY. Why have Law's that don't have to be followed? Sepsis, UTI's and Pressure ulcers are ALL Hospital acquired infections and Medicare is NOT supposed to pay the Hospital. So why did they? Someone is very clearly NOT reading the Medical files before paying the bill's!
Whoever said life get's easier as time goes on is full of it. When a husband and wife are inseparable for over 42 years, life never gets easier. It get's worse. I am no longer a whole person. He IS the Love of My Life and always will be. He always loved me unconditionally. I always came first. He gave me the love I never got growing up. From the time we started dating, we were inseparable. Always together. We knew right away we were meant to be together forever. We did everything together. EVERYTHING! I am so lost without him. I can't stop crying. I just can't fathom how the Hospital we always trusted with our lives could do such a thing. I'm praying this is all a nightmare and my husband will wake up beside me each morning.
I know he was ILLEGALLY put on Palliative Care without our consent. We weren't even told. When it was offered, we refused. It's all in his Medical file, but the Medical board claims they did nothing wrong. We were never told who theses Doctor's and APRN's were. It was all a big secret. What I don't understand is how they can get away with it. It was proven he did NOT have Lung Cancer and I'm fighting to have it taken off his Fraudulent Death Certificate. 
 Criminal charges need to be brought against these death mongers! Euthanasia is "supposed" to be ILLEGAL in NH, but you can't prove it by me. My husband was MURDERED and I won't stop until he gets the Justice he deserves!
Hit Hospitals where it hurt's! Take back their illbegotten money and start auditing them. Make them pay fines. Do something to put an end to Stealth Euthanasia!

According to the new law which went into effect in 2009, Medicare will not pay Hospital bills for patients who have contracted a Hospital acquired infection.

Hospitals are Getting Away With Filing Fraudulent Claims For Hospice/Palliative Care For People ILLEGALLY, UNKNOWINGLY Forced onto Hospice/Palliative Care

I don't understand why Medicare is paying Hospital bill's for people ILLEGALLY put on Hospice/Palliative Care. The Hospital's are NOT filing the claims under Hospice/Palliative Care because the patients were put on it ILLEGALLY. So in turn Medicare is paying them, stating the patient wasn't enrolled in Hospice/Palliative Care such as the case of my MURDERED husband. I was told he wasn't enrolled in Hospice/Palliative Care. That they weren't billed for Hospice/Palliative Care. That's right, he wasn't LEGALLY enrolled, but he WAS Unknowingly enrolled, ILLEGALLY forced onto it. Documentation show's we refused Hospice/Palliative Care. Documentation also show's the name of the Doctor who sent the Hospice/Palliative Care Dr. into my husband's room at the hospital, who proceded to overdose him with 225 mgs. of Morphine a day. Medicare was billed by a Hospice/Palliative Care Doctor and Nurse Practitioner. Both work for the Circle of Life (DEATH) Palliative Medicine. The Doctor is the Director. And even though my husband and I and our family ALL stated we wanted treatment, they gave him none. He told the Oncologist that he trusted that he didn't want to die, he wanted treatment. People (Monsters) that weren't even his Doctors were sent in and killed him.
 I sent in ALL this info as well as the paper that states my husband was put on Hospice. There were no consent forms for Hospice/Palliative Care AND no DNR forms. Medicare STILL doesn't get it. This is plain and simple MEDICARE FRAUD. They are paying for Hospice/Palliative Care because Hospitals know they won't get paid for it if the person wasn't legally enrolled!

A German doctor named Otto Warburg was awarded the Nobel Prize in 1931 for his research proving that cancer cells use a form of non-oxygen metabolism to survive. Mid July 2018 the FDA strengthened their warnings on a group of very troublesome antibiotics which have long been all too commonly prescribed to treat a variety of illnesses such as respiratory and urinary tract infections. In fact I recall warning my own father about these drugs two decades ago! The warnings are extensive and serious – especially so for those with blood sugar issues or those over 60. The warnings are related to mental health issues and severe low blood sugar and as always, are coming much too late for many. This is the very reason our International Coalition for Drug Awareness (ICFDA) was formed. Our mission statement reads:

For the last 17 month's since the death of my husband, I have been trying to be heard. At long last I have succeeded. I have finally proven my husband was forced onto NON-consensual Hospice/Palliative care ILLEGALLY. Treated for Nothing, overdosed, dehydrated and starved to death. He died from UNTREATED Sepsis due to an infection the Hospital never treated him for and we were never told.
Finally, thanks to a letter I wrote to President Trump on the Whitehouse.gov website, my persistence is starting to pay off. Finally, my husband's Murderers may finally be held accountable for their misdeeds!
Medicare is now in the process of going after the money they paid to the Doctor's and a Nurse Practitioner at St. Joseph's Hospital in Nashua, NH, who ILLEGALLY forced my husband onto NON-CONSENSUAL Hospice/Palliative Care that ultimately led to his senseless, untimely death. I am in receipt of ALL documentation including the newly received Medicare summary denying 27 claims made by these Doctor's and Nurse Practitioner. Hopefully rescinding Hospital payments will be next. This story must be told in order to put an end to Stealth Euthanasia in the murder of our loved ones.
For other's who have suffered the same fate as my husband and our family, you must fight back. Our loved ones should still be alive. They did not deserve to be Murdered by the people who should have been helping them. 
My husband wanted to live. He told them straight out he did NOT want Hospice/Palliative Care. He told them he wanted treatment, NOT death. His life meant more than all the money in the world. He was NOT useless and he was NOT dispensable. He was a loving husband, father and grandfather. He was my Knight, taken from me by money hungry unethical Doctors and two Nurse Practitioner MONSTERS! My husband WILL get the Justice he deserves!
I have just one thing to say to the Death Monger's responsible for my husbands death:

The trust running Shropshire's two main hospitals says it has responded to a coroner over concerns relating to a delay in the treatment of a patient with sepsis.

Coroners can make reports to a person or organisation where they believe that action should be taken to prevent future deaths.

A report was issued to Shrewsbury and Telford Hospital NHS Trust (SaTH) relating to an inquest into the death of a patient following a delay in diagnosis of sepsis.
